Authorization for Release Letter

Sometimes an authorization to release of information is needed for a number of different reasons. Generally, those who are requesting the information have pre-formatted forms for you to use. You will notice there is general demographic information. In addition to the demographic information there is also more detailed questions such as the names of the people who are authorized to receive the information. Of course this for is just an example.
