Sifting does make a difference, and I always use a fine mesh strainer. When sifting flour, I also add my other dry ingredients to the top of the flour pile, and it combines the ingredients very well, as well as catches any clumps that have stuck together, such as in baking soda, cocoa, salt, etc.

Sifting makes for a uniform measure, plus it adds air which can make the resulting product lighter. It is important to read the recipe however. A recipe that asks for, say, "one cup of flour, sifted" will use more flour than one that asks for "one cup of sifted flour".
