Antibiotic Therapy Peak and Trough

The principle of antibiotic therapy peak and through is a pharmacokinetic property of any drug. It determines the liberation, absorption, distribution, metabolism and excretion of the antibiotic. This deals with the mean inhibitory concentration (MIC) level of any antibiotic to inhibit intravascular bacterial species propagation throughout the body. The peak refers to the highest point of drug concentration in the blood after liberation and absorption to the body occurs. The trough refers to the lowest point of drug concentration in the blood after metabolism and excretion from the body occurs. Thus, the mean inhibitory concentration (MIC) level is necessary to be maintain throughout the therapy period to prevent recurrences of blood borne bacterial species.

This relates to when you are taking your doses. A peak occurs about an hour after a dose, if taken orally, and the trough is when you are due for another dose. Imagine a graph of how much medication is in your body. It would look like waves...

A PK/PD Approach to Antibiotic Therapy ... PK parameters quantify the serum level time course of an antibiotic. The three pharmacokinetic parameters that are most important for evaluating antibiotic efficacy are the peak serum level (Cmax), the trough level (Cmin), and the Area Under the serum concentration time Curve (AUC).

If we accept the reasonable idea, so far unproven in humans, that the AUC predicts ototoxicity more closely than peak and trough levels, then a group that would be expected to have more ototoxicity than the norm are individuals with poor kidney function.
