As Eustachian tube function worsens, air pressure in the middle ear falls, and the ear feels full and sounds are perceived as muffled. Eventually, a vacuum is created which can then cause fluid to be drawn into the middle ear space (termed serous otitis media).

wow i'm definately going to follow down your route as a cause of my MAV. the only thing that i think can explain my ear pressure and fullness is the muscles spasiming. i just went and had my hearing and balance tests done.

The first picture reveals a pressure of "30" outside the ear (remember, the ear canal is open to the outside) whereas the pressure is only "10" inside the ear. This will cause the eardrum to bulge inward until the pressure outside and inside the ear matches at "30."

The middle ear has a narrow tube which leads down and into the mouth. This Eustachian tube allows the pressure in the middle ear to equalise with the outside world. If this did not happen changes in barometric pressure outside the ear would cause the eardrum to be sucked in and blown out as happens with a simple barometer.
