It's a shrub by nature, tree shape is acheived by grafting onto stem of another kind of willow. All dappled willow same clone. ... Next January I'll prune it down 1/3 as recommended above. Perhaps there will still be a spot in the creeklets to put the cuttings; this is a lovely bush!

There is not a separate tree form versus the bush form. You simply train your bush into a tree form. Very easy. Pick out a central leader and cut, cut, cut. Below is a pic of one of my "trees." It's hit or miss on the rooting.
