There is no difference. A water bug is a type of cockroach. It is different from the more common American Cockroach, which is why some people claim it is not a roach.

People have always differentiated between cockroaches and waterbugs. To me, waterbugs are simply big cockroaches. They both seem the same, except waterbugs are usually much bigger. I would appreciate if anyone could explain the difference between the two bugs, and if "waterbugs" are just as unsanitary as cockroaches.
