In a balanced transmission line, the two conductors have similar properties and are electrically indistinguishable. For example, each wire of a twisted pair has the same voltage drop per length down the line The circuit model for each wire has the same resistance capacitance and inductance per length.

To make a long story short, classic transmission line theory (called "ordinary transmission line theory" in the text above) requires the wave to be launched from one end of the two parallel conductors forming our transmission line.

W2FMI Transmission Line Transformers ... The earliest presentation on transmission line transformers was by Guanella in 1944 1. He proposed the concept of coiling transmission lines to form a choke that would suppress the undesired mode in balanced-to-unbalanced matching applications.

Review of Transmission-Line Theory; The term transmission-line in electromagnetics is commonly reserved for those structures which are capable of guiding TEM waves. Transmission-lines are a special class of the more general electromagnetic waveguide.
