While people are highly unlikely to wink at someone face to face, people do it all the time online. I guess this is simply because the the emote is a simplified version of an otherwise "bold" gesture. Where a wink face-to-face would be awkward, cheesy, and possibly suggestive, the emote is cute, charming, and subtle.
